RN – MEDICAL FLOOR (PRN)

JOB SUMMARY

Responsible as a member of the interdisciplinary team to the Director of Nursing and the attending physician for the provision of skilled nursing assessment, planning, and care in order to maximize the comfort and health of patients and families consistent with Acadia St. Landry Hospitals policies and procedures and Louisiana State Board of Nursing.

QUALIFICATIONS

Graduate from an accredited school of nursing

Currently licensed in Louisiana as a Registered Nurse (RN)

BLS Required – ACLS and PALS Preferred

Ability to assess and respond to the needs of patients and families in varied settings; skilled in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.

Strong written/verbal communication, organizational and customer service skills a must.

Good physical stamina, mental health and personal hygiene habits.

Strong attention to detail

Good oral, written and organizational skills; computer literate and familiar with Microsoft Office software

Understands the healthcare philosophy of care and has empathy for the ill, along with their loved ones.

Proven experience and ability to work in a team-based environment.

Ability to read, follow directions, work independently, and operate mobile documentation device.

Exceptional organizational skills, effective time management, ability to multi-task while still paying close attention to detail

Ability to take initiative and to see projects through to completion.

Ability to work collaboratively and compassionately with other staff in a fast growing, fast paced, and changing work environment.

Team player who shares and participates in the vision and goals of ASLH.

DUTIES/RESPONSIBILITIES

1.       Completes an initial, comprehensive and ongoing assessments of patient to determine healthcare needs.  

2.       Provides a complete physical assessment and history of current and previous illness(es).

3.       Provides professional nursing care by utilizing all elements of nursing process.

4.       Assesses and evaluates patient’s status by: Writing and initiating plan of care, regularly re-evaluating patient needs and participating in revising the plan of care as necessary.

5.       Uses health assessment data to determine nursing diagnosis.

6.       Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.  Includes the patient and the family, if applicable, in the planning process.

7.       Initiates appropriate preventive and rehabilitative nursing procedures. 

8.       Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

9.       Counsels the patient and family in meeting nursing and related needs.

10.   Provides health care instructions to the patient as appropriate per assessment and plan.

11.   Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.

12.   Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.  Records pain/symptom management changes/outcomes as appropriate.

13.   Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.

14.   Communicates with community health related persons to coordinate the care plan.

15.   Teaches the patient and family/caregiver self-care techniques as appropriate.  Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.  Works in concert with the interdisciplinary group.

16.   Provides and maintains a safe environment for the patient.

17.   Participates in on-call duties as defined by the on-call policy when required.

18.   Ensures that arrangements for equipment and other necessary items and services are available.

20.   Supervises ancillary personnel and delegates responsibilities when required.

21.   Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

22.   Fulfills the obligation of requested and/or accepted patient assignments.

23.   Actively participates in quality assessment performance improvement teams and activities.

JOB RELATIONSHIPS

Reports to the Director of Nursing; assists with supervision of LPN’s and CNAs.
